Treatment Options for Glaucoma

Eyedrops are the most common treatment for glaucoma, however about 10% of the patients, eyedrops do not work. In those cases laser therapy or surgery are the common ways to correct the situation and further prevent vision loss.

One of the greatest dangers associated with glaucoma is increased eye pressure i.e. intraocular pressure which damages the optic nerve and can lead to blindness.  In about 90% of the glaucoma cases, the damage progresses slowly and produces no recognizable symptoms until the late stages.Three treatments are used in treating glaucoma.  Medications (eye drops) or oral medications, laser therapy and surgery.

Eyedrops lower the intraocular pressure by blocking excess eye fluid or increasing fluid drainage out of the eye.  In most cases, eyedrops are an effective treatment.  There are some people who do not have success with eyedrops or they forget to use the eyedrops as prescribed.  Some people have difficulty getting the eyedrops into the eye; others stop using them because they don't like the side effects (eye redness or burning).  Cost can become a factor in skipping medications even though the patient knows they are a necessary regimen for the rest of their life.

When eyedrops stop working and do not reduce the eye pressure, glaucoma patients need to have a laser trabeculoplasty.  This is a noninvasive procedure that uses a laser to increase drainage of eye fluids. However, for many patients, this treatment doesn't last forever.  About 40% of the people who have the laser trabeculoplasty, require additional or different medication or some form of surgery within five years.

If the laser treatment doesn't help, the next step is the trabeculectomy surgery. The surgeon will remove a piece of the eye wall (the white of the eye) to create a fluid-drainage channel.  Excess fluid is then absorbed by blood vessels around the eye.

Glacoma implants are another surgical option.  In this procedure, a plastic drainge tube or "shunt" is placed in the front chamber of the eye.  Fluids drain through the shunt and the surrounding tissues absorb the fluid from the plate inside the shunt.  Implant surgery of this nature is effective, but surgeons are reluctant to use shunts in patients who might instead benefit from trabeculectomy.  If the patient has already had one or more trabeculectomy, the surgeon will most likely recommend a the shunt procedure.

Glaucoma: The Importance of Regular Eye Screening

Glaucoma is the second leading cause of blindness and is oftentimes called the "silent thief of sight."  Glaucoma can damage your vision so gradually that you don't notice any loss of vision until the disease is at an advanced stage.  The most common type of glaucoma is called primary open-angle glaucoma and has no noticeable signs or symtoms other than gradual vision loss.

It is extremely important that early diagnosis  be made. It's important to get your eyes examined regularly and make sure  your eye doctor measures your intraocular pressure. Primary open-angle signs of glaucoma include gradual loss of peripheral vision (usually in both eyes) and tunnel vision (in the advanced stages).  Acute angle-closure glaucoma signs and symptoms include severe eye pain, nausea and vomitting (after eye pain), sudden onset of visual disturbance, halos around lights, blurred vision and reddening of the eye.

Don' twait to see a doctor is you notice or become aware of any eye problems. Regular eye exams are the key to detecting glaucoma early enough so you can have successful preventive treatment.  If you are between 18 and 60 years of age, you need to make an appointment with your eye doctor every two years. African-Americans have a much greater risk of glaucoma and need to be screened every three to five year from ages 20 to 29, every two to four years from age 30 to 40, and every one to two years every year after that.

If you have symptoms of a severe headache or pain in your eye or eyebrow and experience nausea, blurred vision or rainbow halos around lights, you may be experiencing symptoms of an acute-angle-glaucoma.  Your doctor would encourage you to schedule an examine as soon as possible.

It is important to know that glaucoma can destroy vision before any signs or symptoms are apparent.  Be aware that if your eye pressure is higher than normal, you are at an increased risk of developing glaucoma.  Be aware that age matters.  Everyone older than 60 is at increased risk of glaucoma. For African-Americans, the risk is much higher and is detectable at a younger age than is the case for the general population. African-Americans should begin to have their eye pressure monitored before age 30.
